Why Most Carbs Suck When Trying to Conceive

First things first

Before getting into the specifics of carbs and fertility, it's worth knowing where a lot of this research comes from: the Nurses' Health Study, running since the 1970s, where thousands of women answered detailed dietary questionnaires every two years, including whether they were trying to conceive and whether they had trouble doing so.

The food pyramid

Carbs get a bad rap thanks to low-carb weight-loss trends, but they're a genuinely important energy source -- your body converts them to glucose, and insulin (released from the pancreas) helps keep blood sugar balanced. Long-term elevated blood sugar can lead to insulin resistance, where cells stop responding well to insulin even as the pancreas keeps producing more. Most diets are pretty carb-heavy, so the real question is whether that matters for fertility, and whether all carbs are equal.

Fertility and pastries, snacks, and sugary drinks

The glycemic index rates how much a food raises blood glucose relative to pure glucose (set at 100). There's convincing evidence that eating more carbs -- especially high-glycemic ones that spike insulin -- raises the risk of infertility from anovulation. The single biggest source of added sugar in the average American diet is soda, energy, and sports drinks, accounting for over a third of total sugar intake. A 2018 Danish study following couples trying to conceive for up to 4 years found drinking just one soda a day was linked to a 20% lower chance of becoming pregnant. An IVF study found women drinking at least one soda a day had 16% lower odds of pregnancy, and seemingly fewer good-quality eggs. The reassuring part: artificially sweetened drinks like diet soda don't show the same fertility association, so you likely don't need to give up soda entirely -- just the sugary kind.

The benefits of whole grains

Whole grains -- quinoa, rye, whole-grain bread, brown rice, whole-grain pasta, oatmeal -- contain antioxidant nutrients like vitamin E and selenium, and help regulate glucose metabolism and reduce insulin resistance, despite being carbohydrates themselves. A study of women undergoing IVF found higher whole grain intake linked to a thicker endometrium (where the embryo implants) and lower odds of miscarriage.

What makes whole grains special

Researchers found the fertility benefit of whole grains is likely driven specifically by one component: the bran, the tough, fiber-rich outer shell (as opposed to the germ, which can sprout, or the endosperm, the grain's energy store). Higher bran intake specifically is linked to better embryo implantation, higher pregnancy odds, and better odds of a live birth -- so adding oat bran to smoothies, yogurt, or homemade bread is a genuinely useful, low-effort change.

Carbs, fertility, and PCOS

PCOS (polycystic ovary syndrome) is the most common cause of anovulation, typically involving small ovarian cysts, higher androgen levels, and insulin resistance -- and it's linked to harder conception overall. Several studies find women with PCOS tend to eat more high-glycemic foods than women without it. If you have or suspect PCOS, it's worth paying close attention to simple carb intake and considering a lower-glycemic pattern, since weight loss through such a diet is linked to more regular periods, better fertility, improved insulin sensitivity, and lower androgen levels. More whole grains, less sugar, is a solid rule of thumb either way -- but especially with PCOS.
